The cardiovascular surgery for being a complex therapeutical intervention possesss high risk for complications in trans and postoperative. With the reduction in the time of internment during postoperative mediate and the return of the patient for its residence they increase the risks for complications in postoperative the delayed one, needing in some cases of reinternao. In accordance with the service carried through for the team of nursing in the adult clinic of a Federal Hospital Reference in Cardiologia was carried through this study that has as object: The nursing consultation as evaluation instrument/accompaniment of the postoperative patient in the delayed one of cardiac surgery. The objectives of the related study are: to present the Service of Nursing in the ambulatorial accompaniment of the patients of postoperative of cardiovascular surgery; to indentificar the main complications of the patients in postoperative delayed of cardiovascular surgery taken care of in the nursing doctor’s office. For accomplishment of this descriptive study, with explicativa goal a quantitative boarding was used involving the systematic collection of numerical information, using statistical procedures. The 692 citizens of the research had been divided in: postoperative; after cateterismo; after implantation of mark-I pass and others. The data had been gotten through handbooks, book of register of patients and official reports of the institution, using two elaborated instruments previously.

The interventions of nursing had been classified in accordance with the NIC. The analysis in the sample that had been carried through 1,221 atendimentos in the clinic in the period of dezembro/2005 to junho/2006. 27% of the patients mention postoperative it of cardiac surgery, being 4 the average number of consultations for patient. 58% are about patients after cateterismo, 5% of patients after implantation of mark-I pass, and 10% of other atendimentos. Some type of complication in the operatria wound was observed in 12,5% of the taken care of patients, being 29% dehiscence in esternotomia, 49% dehiscence in safenectomia and 18% with infection. Amongst the interventions of nursing the procedures most frequent had been: accomplishment of dressings and withdrawal of surgical points. We conclude that to describe the service carried through for nurses the ambulatorial level and to display its experiences and knowledge, becomes important in view of that very this type of accompaniment carried through in the country is reduced, and still is very scarce literature on the subject. Being the operatria wound the main postoperative complication in the delayed one, we elaborate as suggestion a form for accompaniment of wounds who still will be able to supply subsidies creation of a commission of dressings with ends of standardization of the procedure in the institution.
